• Please upload all documents (ID, Insurance Card and/or proof of employment) with your Consent Form
• If you cannot upload, bring a copy of all your documents with you at the time of your appointment.
• Please do NOT share this link.
• Please arrive on time the day of your appointment
• Expect to be there at least 30 minutes.
